It means to replace a harsh word with a less harsh synonym.

Sympathetic Magic

Now should your lungs freeze-seize,
from croop, cough, or sneeze,
call our good friend shem,
his name, it rhymes with phlegm.

"Phem" isn't even a word.

Words that rhyme with shem: ahem, brim, dim, fem, flim, gym, gem, him, hymn, Jim, Kim, limb, mim, nim, quim, rim, slim, stem, trim, Tim vim, whim, zym.
