It seems that the father's waist was getting thinner, so his belt wasn't as tight as it used to be. That's why he cunningly heats the bread without taking it out of the brother's hand; so he can fatten himself back up enough to play the fat dame in the pirate play.
The whole Gregory Peck hallucination was a result of the speaker's not getting enough oxygen to his brain during his breathless episode.
