What I got from this was; Man having sex with a woman, looks down at her vulva, for some reason granny pops into his head, dude just can't get granny out of his head. End result, sad face, and two soft heads
As for the hard and fat part, for me the context was good enough
Other than that it dose the job. That's if I'm reading the poem right
